Region Skåne has eight hospitals where radiologists are working toward one central enterprise imaging system with a PACS and VNA managing production of approximately one million radiology exams per year. Employees who are required to work from home may need to receive allowances for internet service or physical equipment needed to perform their jobs, such as computers, printers and telephones. Just because employees were not entitled to an allowance or work equipment under a permissive remote working policy does not mean the same approach can be taken when mandating remote work. Employees could even be entitled to additional money to compensate them for the employer's use of part of their home for a home office. Diagnostic displays comply with all the required medical image QA standards and guidelines. What’s more, they are engineered for optimal viewing of medical images.

Can Radiologists Work from Home?

I have my laptop connected to two larger external monitors and use the laptop screen for worklists and patient information. This gives me the same three-screen setup as I have at the hospital, so display protocols will work exactly the same. So far, no radiologists here have been reassigned to take direct care of COVID-19 patients, as we have seen, for example, in Italy. Some of my colleagues still have to work at the hospitals, but they have taken several actions to minimize the risk of being infected.

Lunch breaks have been redistributed, meetings are taking place in larger rooms or via Skype, and training of residents is no longer taking place at workstations but in MDT rooms, for example. Artificial intelligence in medical imaging is a topic of much debate and insufficient consensus. A growing number of healthcare facilities are organising based on the Value-Based Healthcare principles. The reasons to facilitate it, such as work-life balance or on-call availability, were hard to balance against high costs and specific technical requirements. With social distancing in the balance, we are seeing a faster pace toward implementation. With artificial intelligence in radiology discussions on the rise, many practitioners wrestle with its accuracy and whether it can be a threat to job security.

Also, we are not seeing as many acute cases coming in, which indicates a tendency among people to delay seeking medical care until the crisis has abated. We are also being affected as a result of more radiologists on sick leave because of enforcements to remain at home even with the mildest of symptoms. Post-covid-19, there are more opportunities for hospitals to allow radiologists to work from home. The added flexibility can increase workplace satisfaction and morale, or act as an incentive to perform the more repetitive, reporting-intensive tasks, such as reporting on screening scans.

A number of well-controlled studies have demonstrated that diagnostic displays improve radiologist performance2. In addition, a diagnostic display system delivers return on investment even after only one year of use as radiologists can come to the correct interpretation more quickly with diagnostic displays3. We asked radiologists how they feel about working from home, during and beyond covid-19.

“You also have the opportunity to stop in and read for hospital radiologists who don’t have the experience in your area of expertise,” Dr. Yuz concludes. There’s the radiologist who performs occasional reading and reporting of exams, in case of an emergency or request for a second opinion, for example. I use the integrated chat functionality in PACS a lot to communicate with my colleagues. It allows us to create groups so we can share and discuss cases, to see where everyone is physically located, and if they are active or offline. It has proven particularly beneficial in these special times—so beneficial, in fact, that even the technologists and radiology nurses now have started to use the chat functionality. In fact, they tend to work more than if they work in a medical or imaging center.

These physicians gain from the perspective of more productivity, independence, and better work-life balance. However, many of the same advantages of radiologists working from home can couple as disadvantages. Mandatory remote work may be preferred by employers and employees under certain circumstances. Even where remote work arrangements serve the interests of both parties, employers must be careful not to hastily implement a mandatory remote working policy. Before any final decisions, employers should fully understand the process for implementing mandatory remote work in each jurisdiction and its implications on managing their human capital. Rather than focusing on the purpose of the benefit, employers should instead look at the legal basis for providing the benefit to employees.

A radiologist’s report on working from home during the COVID-19 crisis

This resulted in some companies making remote work a permanent option. Other companies have taken the opposite position and publicly stated they want their employees back in the office as soon as it is safe to do so. With 30 years of innovation and more than 2,000 installations around the globe, Sectra is a leading imaging IT provider to health systems worldwide. Sectra offers a complete enterprise solution comprised of imaging modules , and a robust VNA.

Though some consumer displays feature similar specs compared to medical displays, they are not fit for home reading. According to the ACR-AAPM-SIIM Technical Standard for Electronic Practice of Medical Imaging, a display used for medical imaging should have a luminance of at least 350 cd/m² 1. This provides radiologists with more visible shades of gray so they can detect subtle details faster. More importantly, the luminance should remain stable over time, meaning luminance output should be measured and controlled, even in a home environment. Sectra’s enterprise imaging portfolio gives you a unified strategy for all your imaging needs, and lets you improve patient outcome while lowering operational costs. Capture, store, access, share, and collaborate around medical multimedia throughout the entire enterprise and beyond.

My workstation is connected to the central system via a secure VPN tunnel administered by Region Skåne. The Internet I use is a regular home fiber connection, with a 100 Mbps download speed and 10 Mbps upload speed. This set-up works very well for me, and my review performance is as good as it is onsite at the hospital. In February this year, we started with a PACS-driven workflow, which considerably facilitates the review since there is no RIS that I need to log into anymore. In addition, speech recognition is completely integrated into the PACS, which makes it very convenient to dictate.

In the first interview, we spoke to Dr Alexander Scholtens, a radiologist at Tergooi in the Netherlands. He explained why the demand for remote radiology work is likely to increase and that artificial intelligence must integrate into their workflow to support this change. Since the start of the global COVID-19 pandemic, teleradiology has experienced explosive demand.
